WELCOME TO THE COVID 19 VACCINE PAGE
We are happy to provide MODERNA, & PFIZER BIVALENT BOOSTER, as well as NOVAVAX BOOSTER vaccines at our pharmacy on an ongoing basis. However, in order to avoid wasting our supplies we request you make an appointment to get the Covid-19 vaccine. The last vaccine or booster must have been at least 2 months ago. ONLY ONE BIVALENT IS ALLOWED AT THIS TIME!!!
How to prepare for your vaccine:
1. Please wear a short sleeved shirt
2. Please bring a valid (in date government issued ID)
3. Bring your Medicare (red white blue card) or your insurance cards
4. Please download and print out the form below and fill it out.
